PPO Directories
  The Plan pays benefits for covered services or supplies received from PPO or Non-PPO providers.  Coverage is generally higher when you use PPO providers. The Plan uses the PPO networks listed below.  Also please see the note about using non-PPO providers below. 
 
  Providence Preferred PPO - Oregon & SW Washington
 

The Providence Preferred PPO is a network of providers - doctors, specialists, hospitals and medical facilities located throughout Oregon and SW Washington.  Providers in the PPO have agreed to charge a discounted rate for covered health care services when you use the network.

  First Choice Health PPO - Washington State (except those in SW Washington)
 

The First Choice Health PPO is a network of providers - doctors, specialists, hospitals and medical facilities located throughout Washington state.  Providers in the PPO have agreed to charge a discounted rate for covered health care services when you use the network.  Click on the category you are interested in viewing to locate a participating provider.

  Using Non-PPO Providers
  If you receive medical care from non-PPO providers, the Plan pays benefits after you meet an annual deductible. Covered services will generally be paid at 60% of reasonable and customary charges after the deductible is met.  You are then responsible for paying 40% of the charges, plus any portion of charges above reasonable and customary charges for your area.
