Your plan requires preauthorization for all inpatient admissions (including hospitals, skilled nursing facilities, chemical dependency treatment facilities, etc.) and certain other services.  Refer to the preauthorization requirements in the Plan Booklet for details.

Your physician should contact Innovative Care Management to obtain preauthorization before you are admitted or before services are provided.

In a medical emergency, you may be admitted to an inpatient facility without preauthorization, but your physician should contact Innovative Care Management within 48 hours or on the first business day following weekend or holiday admissions. Aside from emergency admissions, if you are admitted without preauthorization, the benefit the Plan would normally pay for inpatient care will be reduced by 30% to $2,500 per admission.

Additional days of inpatient care, beyond those initially authorized, must also be preauthorized by Innovative Care Management.
